When you send a message or consultation request, we collect the information you choose to provide: your name, email, optional phone number, treatment interest, optional preferred date, and message. Please do not include sensitive medical information.
How it is used
Your information is used to respond to your inquiry and discuss your requested consultation. A submission is not a confirmed appointment and does not establish a medical provider–patient relationship.
Storage and access
Submissions are stored in our secure cloud database. Public visitors cannot browse submitted inquiries. Information is available to authorized project operators for handling inquiries and maintaining the website.
Spam prevention
A one-way fingerprint of your email is used to limit repeated submissions and reduce spam. It is not displayed publicly.
Third-party content
The location page includes a Google Maps embed. Google may process technical information, including your IP address, when the map loads. Its use is subject to Google’s privacy policy.
Your choices
You can choose not to submit a form. To request access, correction, or deletion of information you previously submitted, contact us and identify your request. Identity verification may be required.
